Register for the Care Information Exchange

Adults can register online using the NHS login or on the NHS app. Children and young people can sign up using our secure online form. If you need help in completing your registration, email the CIE service desk at imperial.care-informationexchange@nhs.net.

Select register now to create an NHS login and set up your Care Information Exchange account.

NHS login

NHS Login is a national service for secure access to digital health and care services with one username and password. To get your NHS login, you need an email address, a mobile phone number and a way to prove your identity – such as a passport, a driving licence, or a European identity card.

NHS app

You can also use the NHS app to register and log into your Care Information Exchange record if your GP practice is in a North West London or North East London borough.

Register now - children and young people

Children and young people can register with our secure online form. Your parents will also need to register if you would like them have access.

What do you need to register?

You will need:

  • an email address for your username and so we can tell you when new information is added to your record.
  • a document to prove your identity

What are the steps?

  1. Fill in the form and book a video call using the link in the form
  2. Show your ID on the video call
  3. Receive an email invitation to activate your account and start using it.

What ID document can I use?

Patients aged 13-17

Passport, provisional drivers licence, or a transport pass with name and photo

Parent or guardian

Driving licence, ID card, or passport and a copy or photo of the young person’s birth certificate

At what age can I get access?

Under 13 year olds

Parents can have access on behalf of their children

13 to 15 year olds

Parents can have access if the young person gives consent. The young person can also have access themselves.

16 and over

The young person now has control over their record. They can give access to their parents if they choose

For young people with learning disabilities 

Parent access on behalf of the young person can continue until the age of 17. Their clinician will confirm if it is appropriate for your child to have access.

Please note, that our clinical staff have a duty to do a safeguarding check for every request to get access to a child's record.
